[DYSPHAGIA] FEES: consents for patients/normals?

I always have a consent form signed when I pass a flex scope. To date, I have 
had no adverse reactions except some mild (nasal) discomfort (I never use 
topical anesthetic).  I have always conducted FEES or flex endoscopy in a 
medical facility with physicians available should they be needed.
