I wonder how individuals are altering their management practices in light of the recent revelation of the dehydrating effects of thickeners and the failure of thickening and chin tuck to prevent aspiration. The authors acknowledge that the ciliary clearance of aspirated thickened fluids probably causes the resultant pneumonia. This is undoubtedly more true of gum based thickeners. It would seem that management, especially of the elderly, requires immediate alteration.
