[Dysphagia] Request for vfss templates

Good people,

We have been having some issues with radiology/speech pathology agreement in
report writing.
We are trying to improve on accuracy and are trying to find a simle way to
remedy

Can any one share one or both of the following:

1. radiology templates for swallow studies (used by the radiologist for
their reports)
2. Simple checklist - provided to the radiologist following the study to
assist them in writing their reports
