[Dysphagia] shaker exercises

The plate is probably an inhibitor to the head raising motions of the Shaker and it should not be implemented in this case. The problem is that this exercise consists of seqeuential and repeated motions and not just normal head/neck motions that are not restricted after the plate is implanted.
