[Dysphagia] dysphagia scale

Has any one found or used a 'successful' rating scale for dysphagia up on
the nursing floors? I have been asked to submit one for our up and coming
computerized system and I don't want to make it up. I need something simple
for all staff to use yet descriptive. Any help would be appreciated.
