Hello fellow list members,
A woman who lives in a rural area asked me what she should use to avoid
inhaling small insects into her trach tube this summer. She has a
Passy-Muir valve that she uses for conversation but cannot tolerate it
for long due to a degree of subglottic stenosis. Do your trach pts use
the stoma covers developed for laryngectomees?

As Eric points out, webs are often associated with sideropenic anemia.
In
pernicious anemia, I wonder if this patient has reached the point of
central
nervous system compromise of the cortex and dorsal horns. Does she have
any
gait/balance problems?
She also is bound to have gastric atrophy, probably motility problems as
well and poor resistance to infection..

I'd like to know if/when people use the term 'absent
swallow', particularly when reviewing a
Videofluoroscopy. If you do use it, how would you
define it and when does it become e.g. a very weak
swallow?

Looking into FEES courses, i know a few posts recently went around about
this- I am aware of Joe Murrays course in Phoenix in Feb... Crary and
Mann course in Orlando in March... and it looks like the next Langmore
course is in July???
Anyone know of any others, if Susan L is doing any others sooner... or
what other ones Joe M is doing???
I'd also appreciate any individual feedback from any of you who have
attended these courses. I need to get into a course pretty soon, but of
course want a good one.

Hi Diane,
I only use the term 'absent swallow' (both at bedside and on
videofluoroscopy) if there is no laryngeal movement in response to
giving a bolus. Otherwise I would call it a (very) weak swallow if there
was some laryngeal movement.

Diane,
When I see such a swallow, I usually term it a "non-functional swallow"
and
state that the person will or will not have difficulty clearing their
own
secretions. This is usually accompanied by a request for a tonsil tip
suction if the person is aware/alert and can do his own oral suctioning.

You can have some laryngeal elevation without a swallow- so absent
swallow
is when there is insufficient hyolaryngeal elevation to transfer a bolus
regardless of tiny efforts which do not produce the physiological event.
