[Dysphagia] FYI: Neuro exam infants and childrenvideopresentations online
I previously worked at a trauma hospital where we had 3 full time SLPs and
per diem coverage. We had speech coverage 365 days a year. We would all
sign up for 1-2 weekend days a month, for which we were paid a per diem
rate. Saturday was a regular day and Sunday was an on call day. If an
order came through the charge RN would call an answering service that would
page the on call therapist. We were required to come in if an eval come
through before 1:00. I found it very beneficial to have coverage on
weekends. Because of all the traumas we did many cognitive-linguistic evals
on weekends and were able to provide info on s/s of mild TBI to pts who were
being d/c pending our eval and ed. I now work at a couty hospital where I
am the only SLP, so there is no weekend coverage. If there is concern on
the weekend about a pt then the MD will either make them NPO or place them
on a puree diet w/ honey thick liquids until an eval can be completed on
Monday.

These days, I do only teaching, writing and consultations all over the
place. I have worked in acute care, geriatric hospitals and facilities for
children and adults with DD.
I have never been able to see the need for weekend coverage at all.

Several years ago I asked colleagues in the San Diego area, but realize I
need an update.
The official policy of our medical center is to cover Mon-Friday, with
the exception of acute rehab, which we cover on Saturdays. Since we are
there on Saturdays, as a courtesy to the patients and physicians, we
complete as many dysphagia evals as possible on Saturdays. Most of the time
this works well, but occasionally we have some dissatisfaction and I'd like
to reply with what the practice is out there. Additionally, we may have to
consider changing our practice depending upon the responses I get.
