What is the latest information on a relationship regarding timing of medication (Sinemet) before a meal in a patient with Parkinsons (my new patient is rapidly declining since admit two weeks ago)? Is it of any benefit?
A quick Google search revealed that a journal article in Dysphagia around 2003 concluded there was no benefit. I believe this has been discussed on the list in the recent past but the archives only go back to May 2004 and I didn't find anything there.

I asked almost the same question of one of the
neurologists where I work. She told me Sinemet will
have no benefit on swallowing function in patient's
with PD.

It's not quite true that Sinemet will have no effect. The problem arises
from the fact that gastroparesis is endemic to PD and the Sinemet eventually
takes too long to pass into the duodenum. If it doesn't reach the duodenum, it
can't cross the blood brain barrier and it is often wrongly assumed that
either the medication is having no effect or the patient has reached end stage.
One of the ways of checking is to do a blood assay to see how much is getting
into the blood stream and, if not enough, increasing the dose. There is a
great deal of literature on this and I am surpised that a neurologist is not
aware of it. There are various mixtures that PD patients have themselves
concocted to overcome this problem the most notable a liquid form of Sinemet that
reaches the duodenum faster. If anyone is interested, I can send the "recipe".

The SipTip cup is somewhat similar. They are available in North America (and will ship worldwide) and have a website which I think is still current www.siptip.com If that doesn't work, try a Google.
They used to (and still may) offer a free sample for clinicians to try.

They're no good for thick liquids but then I don't use thickened liquids
unless absolutely the only option. And for many of the people with PD that
I've worked with, thickened liquids have not helped, or made things worse.
