[Dysphagia] PI Outcome Measures

I am attempting to make a Performance Improvement program for outcome measures in Dysphagia Therapy at an acute care hospital.  I am not looking to use something indepth, as is used in research, but rather something that could be easily implemented into our documentation to show that what we are doing makes a difference.  Does anyone have any ideas that they are presently using?
