[Dysphagia] bronch question

i have a patient who on modified, penetrated down to the levels of vc but did not aspirate (during the evaluation) she showed no pharyngeal sensation during penetration, no cough, throat clear anything...i of course suspsect she's aspirating on thins...her history includes multiple pneumonias of unkown eti. chf and copd.  she is much safer with nectar thickened liquids but refuses them and post mbs developed another pneumonia...recently a mass was discovered in her lung and she is being followed by pulmo. can aspiration be "seen" during a bronch?
