[Dysphagia] difficult case

I have been asked to eval a 44 yo woman w/metatstic breast CA, resp failure, vent dependent w/recurrent pneumonias and respiratory infections. She is adamant that she wants to eat. Her doctor has ordered a swallow eval but the infectious disease and pulmonolgist have documented that she is not to have a swallow eval while she is totally vent dependent. She is unable to tolerate vent weaning for even 5 minutes. Also, she probably has CA in the other breast and will have another radical mastectomy any time now.
The sub acute facility wants me to contact the mds and convince them to do an eval. To be honest, I have not a lot of experience w/vent dependent pts that cannot tolerate any degree of weaning or PMV. I feel very nervous about evaluating a pt w/this hx.
I would appreciate advice from others on how I should progress w/this pt.
