[Dysphagia] Off Topic........

Just an observatrion comment and request:  On October 2nd, I received the
brochure to the DRS annual meeting slated for October 14-16 in Quebec,
Canada.  It is unfortunate that this brochure arrived so late as the topics
and presenters look excellent. Yes, the convention is listed/ topic
described on DRS site, however, does anyone know why the brochures weren't
mailed earlier?  For future meetings, one-two month earlier mailing would
perhaps bring other attendees.  My small irk comment for the day! I feel
better now.

I apologize for this being off topic but I know there is so much knowledge
and experience out there and hoping someone could possibly help.  I'm trying
to create a referral form for physicians to refer to an SLP and wondering if
anyone had one that they or their facility uses that they would be willing
