Just wanted to update everyone who replied to my listing on 15th September.
The patient has gone home today with trache insitu, with PMSV attached. We scoped her and found part of the reconstructive flap was partially occluding her airway- hence the trache remains (further surgery or perhaps laser to remove excess) Pt is not keen for further surgery.
Tongue movement has improved, although not greatly due to lack of tissue. We have a dental clinic in this hospital and the technicians and dentists were very enthusiastic about  building up the pt's denture palate so that her tongue could make easier contact for articulation.
The PEG remains, but the patient is very happy that she can talk and is able to be mostly understood on the telephone.
Overall not such a bad outcome for a patient with so little to work with. Thankyou to all for your help
