[Dysphagia] RE: Dysphagia Digest, Vol 5, Issue 13

I've attended courses with Rona Alexander that I've found very helpful,
most recently the Advanced Clinical Problem-Solving in Pediatric Oral
Motor, Feeding/Swallowing, and Respiratory Coordination Function course.
I've also heard that the ABC Modules at the University of
Wisconsin-Milwaukee that she teaches with Bob Beecher are very good, but
I haven't taken any of them (there are 5, two of which I believe have
now been marketed as video rentals). I've also taken an Introduction to
NDT Treatment of the Infant, which I enjoyed, which focused on entire
body function as well, not solely feeding. As you might expect, none of
the courses I've attended have addressed motor and sensory-based feeding
disorders as separate entities, but instead addressed how the systems
impact each other, and I'm interested to know how the SOS Approach will
integrate the two. Do you work with children as well as adults?

You don't use I.V. you use subcutaneous - hypodermoclysis, overnight 2-3
times a week. Medicare has a code for it and you don't need an RN. Home
caregivers do it all the time. It's the most widely used form of
rehydration
in LTC in the world - outside of the US that is. I find many physicians
won't order it simply because they don't know how to write the order
_(No,
I'm not kidding) - so I write it for them e.g. I recommend
(Route \ fluid type \ fluid amount \ flow rate of fluid \ over what
period of time.)

    I/S 2/3 & 1/3, 1000cc to run wide open q.h.s x 3

(Read: Interstitial, two thirds and one third, 1000 cc's, to flow as
fast as body interstitium will absorb, every night, for the next three
nights.[or however often or p.r.n. etc.]

is similar to traditional intravenous fluid orders such as:

    IV 2/3 & 1/3, 1000 cc @ 50cc/hr saturday and sunday only. Irene

By now, everyone knows I am fighting this issue and each day I become
more and more resolved to make it my mission. The education from Irene
and the positive flavor of the listserve population gives me hope. I
also suspect that there is a commonality that runs state to state (with
the FEDS sticking their nose in it) and that this has become the band
wagon of the day. I would suggest that if enough of us rise up and
challenge them, someone will pay attention. I will either get their
attention in Atlanta at our appeal hearing or die trying. So, feet
don't fail me now! Patti

What is there for children with motor, not sensory, disorders?

There is a course coming up called:
When Children Won't Eat: The SOS (Sequential Oral Sensory) Approach to
Feeding, which focuses on just what you're asking about. The primary
focus is on kids ages 6 months to 6 years, however infants and older
children will also be addressed as part of Special Population workshops
on the third day. Kids with g-tubes and GERD, and Autism Spectrum
Disorders will also be discussed. It will be presented by Dr. Kay Toomey
and Erin Ross (SLP), in Glenview, IL May 14-16, 2004.

Subcutaneously by a thin butterfly needle in the upper thigh, lower
abdomen or, in patients at home who want it during the day, in the upper
arm or shoulder. See the following. The full text article plus patient
handout explaining how it is done is available at the journal website.
Anyone can do it.
American Family Physician, November 1, 2001
 Sasson M; Shvartzman P. Hypodermoclysis:An alternative infusion
technique. Am Fam Physician 2001;64:1575-8
Ben-Gurion University of the Negev, Be'er Sheva, Israel
Hypodermoclysis, the subcutaneous infusion of fluids, is a useful and
easy hydration technique suitable for mildly to moderately dehydrated
adult patients, especially the elderly. The method is considered safe
and does not pose any serious complications. The most frequent adverse
effect is mild subcutaneous edema that can be treated by local massage
or systemic diuretics. Approximately 3 L can be given in a 24-hour
period at two separate sites. Common infusion sites are the chest,
abdomen, thighs and upper arms. The preferred solution is normal saline,
but other solutions, such as half-normal saline, glucose with saline or
5 percent glucose, can also be used. Potassium chloride can be added to
the solution bag if needed. Hyaluronidase can also be added to enhance
fluid absorption. Hypodermoclysis can be administered at home by family
members or a nurse; the technique should be familiar to every family
physician.
