Dear Mary:
I understand the feelings you express. I don't question that the feelings are reality-based and were professionally frustrating for you. I am glad you made a change. I am writing to express a point of view.
With the reductions in revenue opportunities for the industry, companies that have gone "in house" are having their administrative rehab types kind of bird-dog potential for maximizing revenues under Part B of the Medicare system. While that's understandable from their point of view, it is additing yet another stress to the already stressed system. Nobody needs to be having some pencil-pushing type implying that we are not doing the best we can under the circumstances. Unfortunately, that sounds like what was happening at your SNF. Yet, in not working through the situation (and I am NOT criticizing you a bit for your decision), the resident WAS the loser! You didn't make it so, your regional person and/or supervisor did. A number of people are fightin' the fight to be able to deliver rehab at an appropriate level to residents and NOT over-bill. While it's sometimes correct to use more than one code to maximize the care provided, there are other times when it is not. Just do what you do whenever you can. By all means listen to suggestions on better ways to do things. However, ultimately, it is our license and our responsibility.
